I hereby grant permission for my child to participate in activities including photos and recordings, and | am aware that Club Sportif CDL may use any such material in which my child appears for promotional, instructional, educational or commercial purposes.
While CDL’s first concern is always the security and safety of the children, its staff and administration are not responsible for any and all claims of loss, damage, or injury to persons or property, however caused, including negligence, to any party arising directly or indirectly from the child’s participation in squash programs.
In case of emergency, | hereby authorize those responsible for my child’s care to take necessary measures to ensure my child's health and safety.
Club CDL cannot be held responsible for any lost or stolen items.
